Acknowledgment of Risk:
I, the undersigned, acknowledge that participation in dance activities involves inherent risks, including but not limited to physical injury, strain, or discomfort. I understand that these risks are part of the nature of dance and voluntarily accept these risks for my child’s participation in the trial class at Studio D Dance.
Medical Conditions and Physical Limitations:
I have disclosed any known medical conditions, physical limitations, or allergies that may affect my child’s ability to participate in dance activities. In case of an emergency, I give permission for my child to receive medical treatment if deemed necessary.
Release and Waiver:
In consideration of allowing my child to participate in the trial dance class, I hereby release, discharge, and hold harmless Studio D Dance, its owners, instructors, employees, and affiliates from any and all claims, liabilities, or causes of action arising from my child’s participation in the class. This includes any injury or damages sustained by my child, whether caused by negligence or otherwise.
Photographic Release:
I grant Studio D Dance permission to photograph or video my child during the trial class for promotional purposes, including but not limited to social media, website content, and marketing materials. I understand that these images may be used without compensation to me or my child.
Cancellation Policy:
I understand that if we need to cancel the trial class, we must provide at least 24 hours' notice. Failure to cancel within this period will result in forfeiting the trial class.
Consent:
By signing below, I confirm that I have read and understand this liability form, and I agree to its terms.
